Do not worry how the form looks in the email you send to me for now as I can get the information I need from it and will you send a confirming email of your choices after I transfer your selections to my hand written list. Google, as usual, works great with no killer problems and is the preferred method to use.

Using Firefox works ok and gives me the same information but when you get down to the text boxes you can not click the text boxes to get your curser in them to add information, you must use TAB and SHIFT TAB to move around to the different text boxes.

And, as usual, Microsoft Internet Explorer does not work at all so you will have to send me an email with your choices in it as described on the bottom of link 5.
